Under fault conditions, accessible metal parts may become live and, if touched, give the user an electric shock. The product also failed the electric strength test and breakdown occurred before full test voltage was applied. No instructions were given for installation or initial use of the product.
Corrective Action: OPSS is requiring the recall of the product from end users.
